It is about the pain of watching someone you love slowly self-destruct one drink at a time until they are a shell of their former selves.

It is alcohol that is the vampire that sucks the very life out of an individual suffering from alcoholism. This song is about a person giving you their experience of the hopeless and helplessness having to stand by a watch a loved one deteriorate before eyes.

He's been hurt in the past too, and still hasn't resolved all his issues. I think the whole reference to the drunk and passed out thing is just an indicator that it was a recent fight, and he's probably out drinking his sorrows away.
